We want all partners in Somerset's health and care system to work together as one. When organisations work separately, it can be confusing and tiring for people using their services. By working together, partners can do things more efficiently, save time and money, and treat people as whole individuals.
In Somerset, this teamwork includes many groups. They are our volunteer organisations, our community groups, our faith groups, our social enterprises, our health services (including your local GP), social care, public health, housing, police, education, and even fire and rescue.
Technology and data are very important in helping teams work well together. This can mean using shared computer systems, creating ways for people to share knowledge, or making sure information flows smoothly across the system. This helps us provide targeted care and understand the health of our whole community better.
